The coach or manager of a player or a coach receiving the Game Misconduct and / or Match penalty must file this report of the incident within twenty-four (24) hours. It is automatically sent to the DVHL Commissioner when you hit the "Submit" button. All Match penalties involving Abuse of Officials must also be reported to the Atlantic Affiliate Vice President of Protests, Suspensions and Appeals Ron Goodman. You can forward your return acknowledgement email to Ron @ rgoodmanvp@yahoo.com. This report is due for ALL Match & Game Misconducts received in League, Non-League, Playoff and Tournament games. It is also to be filed for violations of the five (5) penalties in a game rule for players and fifteen (15) penalties in a game for a coach. Failure to file this report within the time specified shall result in the imposition of a fine of $30.00 on the organization of which the manager/coach who failed to file the report is a member.
